District nurses are specially trained to care for people who are housebound due to serious illness, age or chronic disability. They offer a service seven days a week, liaising closely with the GPs and practice nurses. Our district nurses are based in Soho but come to the practice monthly.

 

Health Visitor Team

​

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing. Our health visitor is based at Lisson Grove Health Centre but attends the practice monthly. They also hold a clinic at Marylebone Health Centre so sometimes see patients there.

Care Navigator (female)

​

Care Navigators are responsible for supporting GPs through providing care coordination for the whole population. This includes, for example, being responsible for intelligent tasking and patient referrals, booking of transport and equipment where required and delivering targeted public health messages to the full GP practice list. In addition, CN will work with the relevant agencies of the health and social care system to ensure coordinated and effective delivery of the patient’s care plan for those patients identified by the GP.
